Your benefits are an important part of your total compensation; therefore, we invite you to familiarize yourself with the details of these plans.

Who is Eligible and When?

All regular full-time employees working 30+ hours a week are eligible to enroll in health insurance. Eligibility for newly hired employees is the 1st of the month following 60 days of employment. New employees will have 30 days to elect medical coverage. Current, eligible employees who have elected to waive coverage are able to enroll during Open Enrollment. Only if they experience a qualifying event (including losing coverage elsewhere, getting married, having a baby) are they able to enroll outside of the open enrollment period.

How Do I Enroll?

Newly hired, regular full-time employees will enroll for health insurance benefits during the new employee orientation period. Eligibility begins the 1st day of the month, following the 60th day of employment.

Waiving or Declining Health Insurance

If you are eligible for health insurance and will not be enrolling because of alternative coverage, or choose not to have insurance, a waiver form must be signed during the enrollment period. If you elect not to enroll, you may not join the plan until the next enrollment period, in late fall of 2025, unless there is a “qualifying event.”

All employees should be aware of possible Federal tax penalties for declining Carlisle’s health insurance plan enrollment, as well as alternatives for health insurance available through the Health Insurance Exchange. What happens if I leave Carlisle?

Under certain circumstances, you and your dependents may continue to participate in health coverage, dental insurance, vision insurance and the Medical Flexible Spending Account through the Consolidated Omnibus Budget Reconciliation Act. COBRA is a federal guarantee of the continuation of health insurance plan coverage after employment ends, which allows you to remain on medical and dental coverage, at the employee’s expense, for up to 18 or 36 months, depending on the circumstances. Former employees who enroll via COBRA will be responsible for the full cost of the monthly premiums.

Domestic Partnership: An employee’s domestic partner and domestic partner’s dependent children can enroll in the Carlisle health, dental and vision insurance plans.

Health Plan Overview

The major cost sharing components for Carlisles’ plans are copays, deductibles, coinsurance, and Out-of-Pocket maximum amounts. All accumulators are tracked on a calendar year (January – December)

A copay is an amount set under the medical plan that the member pays for covered services – such as an office visit –  or prescription drugs.

The EPO 1500 has copays for virtual visits, office visits, urgent care and prescriptions drugs.

Deductible is the amount of deductible-eligible covered medical expenses (i.e. inpatient, outpatient, imaging, lab, ER) the member pays before Allied pays benefits.

The EPO 1500 has a $1,500 deductible for single coverage. $3,000 for dual coverage and $4,500 for family coverage.

Coinsurance is the amount the member pays for deductible-eligible services after meeting the deductible.

The EPO 1500 has zero coinsurance for medical services and 20% coinsurance for durable medical equipment.

Out-of-Pocket Maximum refers to the most you will pay for covered expenses under the plan. Copays, Deductibles and Coinsurance all accumulate toward the OOP Max.

Where you receive your care can impact how much you pay:

  • Urgent Care Centers, such as Convenient MD are generally less expensive than hospital emergency rooms
  • Stand-Alone labs such as Quest Diagnostics on West Street in Keene are generally less expensive than hospital labs.
  • Ambulatory Surgical Centers in Concord, Bedford, Manchester and other locations in NH give members an opportunity to pay lower fees for certain elective surgeries/services.

Flexible Spending Accounts

Flexible Spending Accounts (FSA)

What is a Medical Flexible Spending Account? A Medical FSA is a tax-advantaged employee benefit that allows eligible employees to voluntarily set aside pre-tax dollars through equal payroll deductions to be used for eligible health care expenses.

Who is eligible?

Carlisle offers both health (FSA) and dependent care (DCA) flexible spending
accounts. Employees may contribute pre-tax dollars into these accounts to help offset eligible medical expenses or dependent care expenses. New administrator this year! The FSA & DCA will be moving from Voya to Allied.

Eligible employees may elect to contribute up to $3,300 during the plan year, January 1, 2025-December 31, 2025.

 Medical FSA Basics

  • Medical FSAs are used to set aside money for planned or reoccurring qualified medical expenses that are not covered by insurance, such as dental care, vision care, contact lenses, co-pays, deductibles, out-of-network expenses.     • Under the Coronavirus Aid, Relief and Economic Security Act (CARES Act), the definition of a qualifying medical expense now includes certain over-the-counter (OTC) medications and products. For some examples of these products, see the “Expanded FSA Expense” flyer.
  • You have full access to your total election amount on the 1st day of that plan year. You do not need to wait until your FSA payroll deductions equal the amount of your qualified medical expense.
  • Most transactions can be paid at the point of sale with a debit card pre-loaded with your total FSA amount for that plan year.
  • You have 90 days from the end of the plan year in which to submit any claims. For the 2025 plan year, you will forfeit any money over $660 that is left at the end of the plan year in your Health FSA account. If you have $660 or less in your health FSA account those funds will automatically be carried over into the subsequent plan year.

What is the most I can contribute?

  • $3,300 for plan year 1/1/2025-12/31/2025
Dependent Care Flexible Spending Account Basics

The Dependent Care Assistance Plan (DCAP) Account is designed to help you pay for child care services for a child who is under age 13 or care for a disabled Spouse or Dependent when those services make it possible for you (or you and your Spouse if you are married and/or your Spouse is not disabled or a full-time student) to work.

You qualify for this benefit if:

  • You are a single parent; or
  • You have a working Spouse; or
  • Your Spouse is a full-time student for at least five months during the year while you are working; or
  • Your Spouse is disabled and unable to provide for one’s own care.

A DCA allows employees to set aside pre-tax dollars to pay for qualified dependent care expenses. Funds can be used to pay for day care, preschool, or other care for qualified tax dependents. To qualify for a dependent care FSA, the IRS requires that the dependent care is necessary for you or your spouse to work, look for work or attend school full-time.
In 2025 the maximum amount you may contribute to the dependent care FSA is $5,000* (if single or married & filing jointly) or $2,500* (if married & filing separately). Please note that the DCA is 100% employee funded and is only available to use as monetary contributions are made.

Eligible Daycare Expenses :
•Childcare or Adult Care by a licensed childcare facility, for children under 13 years of age who qualify as tax dependents.
•Childcare or Adult care for children or adults of any age who are physically or mentally unable to care for themselves and qualify as dependents.

Carlisle offers it’s employees a Dental Plan through Northeast Delta Dental. The plan covers Preventive Care, Basic, Major Restorative procedures and Orthodontics.
